The Oracle at Delphi was the most famous oracle in ancient Greece. She would go into the gave and babble for awhile. Then the priests of Apollo would interpret what she said and tell you. Nearly every major decision was made with the oracle's input. Archaeologists found that the cave she used once leaked a gas from inside the earth which caused her hallucinations, which was the source of the "prophecy". After the vent where the gas came from was closed up by an earthquake, the oracle stopped working.

EDIT: Spelling mistakes.

EDIT2: Also, the prophecies were always worded very ambiguously. Depending on how someone interpretted it, or even on where the put punctuation, a prophecy could spell doom or success. Thus, if the opposite result from the interpretation occured, the priests could say that was what it actully meant. So the oracle was never wrong!
